Before we arrange an assessment we will require some information about the student and about the reasons for the assessment being sought. This enables us to prepare for the assessment and to select the most appropriate assessment instruments. We also require a signed consent to the assessment from the parents or guardians.
The referral form and consent form are attached. These can be downloaded and sent to us by mail or fax. When we receive them we will be in touch with you to arrange a date for the assessment. We aim to make all of our appointments within 4 weeks of receiving the referral forms.
The assessment can be arranged for a convenient location - our offices, the student's school or home.
